At Convergence Zone Transportation (COZO), our mission is to deliver high-quality non-emergency medical transportation (NEMT) services at fair and competitive rates. When you complete our Request for Quote form, you provide us with essential information about your transportation needs, level of personal assistance required, and any special accommodations. This helps us tailor our services to ensure your experience is safe, comfortable, and exceeds expectations.

Unlike a one-size-fits-all approach, we recognize that every client’s circumstances are unique — including their health needs, personal situations, and financial considerations. At COZO, you are never just a number. You are treated with the personal attention and understanding you deserve.

Factors used to develop your personalized quote:

1. Type of Service - depending on if you are requesting wheelchair service or ambulatory services the price varies. 

2. Mileage - For ambulatory and wheelchair services, the first 20 miles are included.  For stretcher services, the first 15 miles are included.  For bariatric services, the first 10 miles are included.  Any mileage beyond the initial mileage will incur an additional per-mile fee.  For trips longer than 50 miles loaded, there is a long-distance surcharge applied to the extra miles.  There are additional fees for traveling outside our designated service area. 

3. Companion Fee - Additional fee if you need to travel with an attendant whether one of COZO's certified companions or your own attendant.

4. Additional Assistance Needed - such as help getting on and off the vehicle, assistance in/out of facilities, moving of equipment (i.e. wheelchairs, oxygen tanks, stretchers, and heavy lifting).  To get an accurate quote please clearly state your personal needs. 

5. Wait Time - we do calculate mileage, but if you request COZO to wait during your appointment or errand, there will be a wait charge.  For our trips, we do calculate as a round trip and a pickup time will be scheduled.  From the time your appointment is scheduled COZO gives you 1 hour of complimentary wait time after 1 hour fees will begin to incur. 

6. Weekends, After Hours, & Holidays - Rides scheduled during weekends, after hours, and holidays will have a higher rate. 

7. Short Notice Bookings -  we require a minimum of 48 hours to schedule your trip.  If you schedule your trip 24 hours or less before your schedule trip a surcharge will be added.  For these types of booking, we prefer the client to call us, so we can address your transportation quickly.

When you book a trip with Convergence Zone Transportation we trust our clients will uphold their booking.  We do require a deposit to schedule trips.  Your deposit will be placed against the total cost of trip once your trip is completed.  Our deposit rates are the same as our No-Show/No-Call rates.  If you do not complete your trip, you will forfeit your deposit.  No-Show/No-Call fee: $75 (Regular), $150 (weekends/after hour), $200 (holidays)
